Ulcerative colitis (UC) is a chronic inflammatory disease that affects the colon, impacting the quality of life of individuals at any age. There is a wide spectrum of clinical situations, and recent advances in the pathophysiology of UC have led to the development of new, more effective and safe therapeutic agents. This article presents the case of a 31-year-old male patient who was admitted to the outpatient clinic and subsequently hospitalized for a 21-day history of semiliquid stools that progressed to liquid stools up to 18 times on one occasion, with rectal bleeding, accompanied by abdominal pain. During his hospitalization, the diagnostic impression was: chronic diarrhea and rule out enteroperitoneal TB. Auxiliary laboratory tests were performed with the following diagnoses upon admission: parasitosis by Entamoeba histolytica and E. coli infection, and moderate anemia. With a stationary evolution, the possibility of ulcerative colitis was subsequently considered, and a colonoscopy with biopsy was scheduled for further study. A colonoscopy was performed, which revealed ulcerative lesions from the rectum to beyond the splenic flexure, suggesting a diagnosis of ulcerative colitis. A biopsy study confirmed the diagnosis of extensive ulcerative colitis.
