Abstract

Objective: To determine the relationship between post-transfusion immune reactions and the extraction time of packed red blood cells in transfused patients at the Central Military Hospital of Peru in 2024. Materials and Methods: This was an observational, analytical, cross-sectional, and prospective study. Data was collected over a twomonth period at the Central Military Hospital of Peru “Coronel Luis Arias Schreiber” in Lima. Information was obtained from the Hemotherapy and Blood Bank service, as well as from the
services where patients were hospitalized, resulting in 136 samples. These were processed using the direct Coombs test and entered into a database. Results: It was demonstrated that 38.5% of transfused patients who received packed red blood cells older than 15 days tested positive for the direct Coombs test. According to the positive post-transfusion immune reactions, 38.8% of packed red blood cells older than 15 days were positive at 1+, followed by 26.5% with 3+ and 22.4% with (+/-). Ultimately, 3.7% of all transfused
patients died, and these were those who received packed red blood cells with an extraction time greater than 15 days. According to the correlation test between post-transfusion immune reactions and the extraction time of packed red blood cells, the correlation was very low and positive (Rho: 0.126; p: 0.072). Conclusion: The relationship between post-transfusion immune reactions and
the storage time of packed red blood cells is directly proportional at a very low level.
